ABOUT THE TRANSITIONS PROGRAM What is Transitions? Transitions is a program that offers assistance, encouragement and support to individuals and their families who have a life limiting illness with an expected prognosis of a year or less. Patients and their families are treated with courtesy and respect for their own personal belief systems and values. All information is treated confidentially. Who is Eligible? Individuals are eligible for the supportive services of Transitions with the consent of their physician and regardless of their current medical treatment. How to Get Help Referrals can be made by family, friends, doctors, nurses, and other professionals. Call 815-756-3000 to start the process. TRANSITIONS SERVICESTransitions Coordinator The Transitions Coordinator is a client advocate who will work with the client and family. Together they will identify needs and the coordinator will offer assistance to fulfill those needs with referrals and links to community resources. The Transitions Coordinator is available to offer support and answer questions by phone or a home visit. Transitions Volunteer Transitions uses specially trained volunteers who provide extra support to the Transitions client and their family. Volunteers offer:
